Vitiligo is a disfiguring condition that can cause considerable psychological distress to patients. Vitiligo lesions on acral areas and joints are considered difficult to treat, and they are unsuitable for surgical treatment because of their poor response. There are few studies on the management of those lesions with noncultured epidermal cell suspension transplantation.
Acral lesions of vitiligo are usually resistant to conventional lines of treatment as well as surgical interventions.
We retrospectively analysed data for patients who had undergone non-cultured epidermal cell suspension transplantation for treatment of vitiligo. In total, 36 patients with 80 lesions over acral areas and joints were reviewed: 33 patients had generalized vitiligo, while the remaining three patients had focal vitiligo, and they had been followed up for 6-18 months.
RESULTS:Of the 80 treated lesions, 51 had regained > 75% repigmentation and 23 had regained 50-75% repigmentation. The remaining six lesions, which were all no the distal fingers or toes and the ankle joint, had a poor response.

Acrofacial vitiligo (AFV) is currently classified as a form of generalized vitiligo (GV), under non-segmental vitiligo (NSV). However, there is no scientific evidence of these being distinct diseases as they may evolve in time in to generalized vitiligo. AFV can be seen with either a limited involvement of the lips, tips of the fingers/toes and penile shaft as the ‘lip-tip’ form; or, acral vitiligo (AV) in various stages of centripetal progression to the trunk.

The 308-nm excimer laser showed equivalent efficacies to 308-nm excimer lamp control and NB-UVB control concerning ≥75% re-pigmentation rate of vitiligo patches. More studies with high methodological quality, low risk of bias and more sample size are needed to confirm the conclusion.

Since you’re more sensitive to sunlight when using psoralen, you should protect the rest of your body with sunscreen. You can get PUVA treatment several times per week in your doctor’s office.
Depigmentation
When repigmentation of the white patches fails, depigmenting the rest of your body is another treatment option. Depigmentation is reserved for those who present with vitiligo on more than half of their body. The goal is to match your normal pigmented skin to the areas affected by vitiligo.
A medication called monobenzone is applied to your skin to lighten the pigmented areas. This topical medication can bleach the skin of other people who may come into direct contact with you. Therefore, skin-to-skin contact must be avoided for two hours after treatment.
Noninvasive management of vitiligo can include makeup and sunless tanners. Tanning doesn’t help restore pigment loss from vitiligo, according to the NIAMS. Tanning makes the difference between normal and depigmented skin tone more obvious. You should use sunscreen with UVA and UVB protection to protect your skin.
Surgery Options

In severe cases, you may elect to have surgery to try to restore normal skin tone. Skin grafting takes healthy skin and places it in an area that has been affected by vitiligo. Such surgical procedures are possibilities for people whose vitiligo hasn’t improved despite several years of consistent treatment. The surgery is expensive, rarely covered by insurance, and can cause scarring.
